Book an Appointment

We provide the convenient option for you to book an appointment online through our website or by calling our clinic directly.

Specify the Type of Service you are looking for: You should always specify if you’re looking for general chiropractic care, pregnancy or paediatric care, or have a specific issue like back pain or scoliosis just to name a few.



Complete New Patient Forms

Health History Form: This is essential for understanding your current health status and medical background.


It typically includes:

Personal details

(name, contact information, date of birth)

Details of the primary issue

(e.g., back pain, neck pain, headaches)

Medical history

(previous surgeries, injuries, or chronic conditions)

Current medications and treatments

Lifestyle factors

(e.g., exercise, work habits, posture)

Allergies, if any


Consent Forms: You may also need to sign a consent form, acknowledging that you understand the chiropractic treatments and agree to receive care.




Health Insurance Information

(If Applicable)


Health Insurance:

If you are unsure, it's best to contact your health insurance fund first).


Health Fund Rebates:

If you have private health insurance, you are able to claim rebates immediately through the clinic via our swipe and pay electronic claiming systems with tyro.



Follow-Up Care & Ongoing Support

Scheduled Follow-Up Appointments:

Based on your chiropractor’s recommendations, you may be asked to schedule follow-up visits for ongoing treatment.


Home Care Instructions:

We may provide you with exercises or lifestyle tips to help support your recovery between our appointments.
